Position Summary:
Holy Cross Hospital Silver Spring is seeking a dedicated and experienced Registered Nurse (RN) Case Manager to join our Care Management team for a long-term assignment. This role is essential in facilitating high-quality, cost-effective care and discharge planning for hospitalized patients.

Key Responsibilities:
  • Coordinate discharge planning processes for patients across the continuum of care.
  • Perform comprehensive assessments and develop individualized care plans in collaboration with the interdisciplinary healthcare team.
  • Ensure timely transitions of care while advocating for patient needs and goals.
  • Communicate and collaborate with physicians, nurses, social workers, and outside agencies to facilitate appropriate services and resources post-discharge.
  • Monitor and document patient progress toward achieving discharge goals and adjust plans as needed.
  • Ensure compliance with hospital policies, CMS guidelines, and accreditation standards.
  • Act as a liaison between patients, families, and healthcare providers to address concerns and promote understanding of care processes.

Requirements:
