Northgate Health & Rehabilitation Center is seeking an experienced Registered Nurse (RN) to join our team as an MDS Coordinator. This is an outstanding opportunity for an organized, detail-oriented nurse with experience in skilled nursing, long-term care, rehabilitation, and Medicare reimbursement.

Position Overview

The MDS Coordinator (RN) is responsible for coordinating and completing the Resident Assessment Instrument (RAI) process, including Minimum Data Set (MDS) assessments, care planning, Medicare documentation, and clinical reimbursement activities. This role works collaboratively with nursing leadership, therapy, physicians, and the interdisciplinary team to ensure accurate documentation, regulatory compliance, and optimal reimbursement.

Responsibilities
  • Coordinate and complete MDS assessments in accordance with CMS guidelines and federal and state regulations.
  • Ensure timely and accurate completion of the Resident Assessment Instrument (RAI) process.
  • Develop, coordinate, and participate in interdisciplinary care plan meetings.
  • Review resident documentation to ensure accuracy, compliance, and appropriate clinical reimbursement.
  • Collaborate with nursing, therapy, social services, dietary, and other departments to ensure comprehensive resident assessments.
  • Monitor Medicare, managed care, and reimbursement documentation.
  • Maintain survey readiness and support compliance with all regulatory requirements.
  • Educate nursing staff regarding MDS documentation and assessment requirements.
  • Monitor quality measures and participate in quality improvement initiatives.
  • Assist with additional clinical responsibilities as assigned.
Qualifications
  • Active Alabama Registered Nurse (RN) license required.
  • Previous experience as an MDS Coordinator, RNAC, Clinical Reimbursement Coordinator, or MDS Nurse preferred.
  • Skilled nursing facility, rehabilitation, or long-term care experience required.
  • Strong understanding of MDS 3.0, RAI process, Medicare reimbursement, PDPM, and CMS regulations.
  • Excellent assessment, documentation, organizational, and communication skills.
  • Ability to work collaboratively within an interdisciplinary healthcare team.
