Job description

Heritage Pointe of Huntington has an opportunity for an Infection Prevention & Wound Care Nurse. This role is an important part of our clinical team, supporting quality resident care through infection prevention, wound management, staff support, and regulatory compliance.
We are seeking a compassionate, organized, and clinically skilled nurse who is passionate about improving resident outcomes and collaborating with interdisciplinary teams.
Position Responsibilities

  • Coordinate and oversee the facility's Infection Prevention and Control Program.
  • Monitor, investigate, and track infections while implementing prevention strategies.
  • Lead infection surveillance, reporting, and regulatory compliance activities.
  • Assess, develop, and monitor individualized wound care plans.
  • Collaborate with physicians, nursing staff, therapy, and other disciplines to promote optimal wound healing.
  • Provide education and guidance to staff regarding infection prevention and wound care best practices.
  • Participate in quality improvement initiatives, audits, and performance improvement activities.
  • Maintain accurate documentation and ensure compliance with state and federal regulations.
  • Work on the floor to help with admissions/discharge or cover as needed. Will include on call rotation

Experience:
LTC minimum 2 yrs preferred
RN/LPN
