As a Clinical Liaison/Patient Care Coordinator, you will:

Work as part of our local sales team and report to a Market Executive of Sales

Coordinate care from Hospitals and SNFs in the territory

Meet with the patients at the bedside to discuss home health services, disciplines and what to expect at home. home.

Support operations teams by ensuring they have the resources, information, and coordination needed to achieve a successful start of care.

Assist with sales calls as needed to support Account Executive in addressing client needs and advancing business objectives.

Use your skills to make an impact

Required/Preferred Experience/Skills:

LPN, LVN or RN license is required.

Knowledge of governmental regulations, Medicare eligibility requirements, comprehensive understanding of potential care plan needs for the patient and coordination of necessary resources.

Three years clinical experience preferred.

Travel within assigned geographical territory is required.

This role is considered patient facing and is part of Human's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.
