• Conduct group and individual therapy sessions, fostering a therapeutic environment that promotes healing and understanding.
• Perform thorough clinical assessments and develop effective treatment plans tailored to individual patients.
• Initiate and manage discharge planning from the day of patient admission, ensuring a smooth transition to aftercare.
• Communicate actively with the Utilization Review and Business Departments regarding patient authorizations and billing processes.
• Facilitate effective follow-up appointments by confirming discharge plans with patients and their families.
• Maintain continuous communication with referral sources and outpatient providers to ensure seamless continuity of care.
• Assess patients for risks, including suicidality or potential harm to themselves or others, and manage crises effectively.
• Support milieu staff in their interactions with patients, ensuring a unified and supportive therapeutic atmosphere.
• Participate actively in clinical outreach initiatives and program tours to promote service engagement and community involvement.
• Complete required documentation, including initial assessments, SMART treatment plans, and clinical notes, in a timely manner.

This role primarily focuses on diverse client populations experiencing various mental health challenges. You will engage with individuals from various backgrounds and demographics, providing culturally competent care that acknowledges and respects their unique situations. Your work will directly impact their mental health journeys, equipping them with the necessary tools for coping, resilience, and recovery.

• A Master's degree in Social Work, Counseling, Psychology, or a related mental health discipline is required.
• An active and independent license as a Licensed Clinical Social Worker (LCSW) is necessary.
• Proficient written and verbal communication skills, demonstrating an ability to convey complex information clearly.
• Ability to model appropriate boundaries and demonstrate a strong ethical framework in therapy.
• Familiarity with electronic health record systems and Microsoft Office 365 is preferred.

• Proven ability to manage documentation effectively, including clinical notes and treatment plans.
