Job description

Wake County Public Health is looking for a Nurse Family Partnership (NFP) Nurse Home Visitor (NHV) to join the team! 

Are you passionate about supporting first-time parents,improving maternal and child health outcomes, and making a lasting impact inyour community? As a Nurse Home Visitor (NHV) with the Nurse-FamilyPartnership program, you'll build meaningful relationships with pregnant womenand their families, providing evidence-based nursing care, education, andsupport from pregnancy through early childhood.

In this role, you'll deliver comprehensive community healthnursing services to eligible families while maintaining the highest standardsof clinical practice and fidelity to the Nurse-Family Partnership model. TheNurse Home Visitor reports directly to the NFP Nurse Supervisor and plays avital role in helping families achieve healthier futures.

What you'll do as a Nurse Home Visitor: 

  • Develops and maintains community relationships to support client referrals
  • Conducts community outreach of potential program clients
  • Collaborates with all multidisciplinary team members and with a patient focus to implement the identified plan of care and facilitate and maximize client health care outcomes
  • Consults and collaborates with the Nurse Supervisor and the NFP team during case conferences in the interest of providing optimal client services
  • Monitors the patients progress, intervening as necessary and appropriate to ensure that the plan of care and services provided are client focused
  • Facilitates postpartum/prenatal depression screening/counseling with clients and makes referrals for ongoing services as appropriate
  • Maintains a positive relationship with physicians, nurses, other health providers, social workers, clients and their families to ensure that each client receives optimum care
  • Interacts with other members of Nurse Family Partnership program team in ongoing program development implementation and evaluation
  • Maintains knowledge of applicable rules, regulations, policies, laws and guidelines that impact nursing practice
  • Stays informed on current health care developments to provide safe, quality nurse home visiting services
  • Completes all required NFP education
  • Actively engages in skill building to meet all NFP Home Visitor competency requirements
