Job Title: Long-Term Care Registered Nurse (LTC RN)
Job Summary:
The Long-Term Care Registered Nurse is responsible for delivering high-quality nursing care to residents in a long-term care setting. The RN assesses residents' health status, develops and implements individualized care plans, administers medications and treatments, monitors changes in condition, and collaborates with physicians, families, and interdisciplinary team members to ensure optimal resident outcomes.
Key Responsibilities:

  • Perform comprehensive nursing assessments and document findings accurately.
  • Develop, implement, and evaluate individualized care plans.
  • Administer medications, treatments, and IV therapy as prescribed.
  • Monitor residents for changes in physical, emotional, and mental health.
  • Respond promptly to medical emergencies and changes in condition.
  • Supervise and delegate duties to Licensed Practical Nurses (LPNs) and Certified Nursing Assistants (CNAs).
  • Maintain accurate and timely medical records in accordance with facility policies.
  • Educate residents and their families about health conditions, medications, and treatment plans.
  • Collaborate with physicians, therapists, social workers, and other healthcare professionals.
  • Ensure compliance with state, federal, and facility regulations.
  • Participate in quality improvement initiatives and infection prevention practices.
  • Advocate for resident safety, dignity, and rights.
Qualifications:
  • Active Registered Nurse (RN) license in the applicable state.
  • Graduate of an accredited nursing program (Associate or Bachelor of Science in Nursing preferred).
  • Current CPR/BLS certification.
  • Previous long-term care or skilled nursing experience preferred.
  • Strong assessment, critical thinking, and communication skills.
  • Ability to manage multiple priorities in a fast-paced environment.
  • Proficiency with electronic health record (EHR) systems.
