Job description

PRIMARY OBJECTIVES

1. The escort accompanies the resident to clinics, private physician’s offices and other treatment facilities in order to provide seamless and safe care and service.

Pay Ranges from $15- $17

Monday - Friday

While performing the duties of this job, the employee is regularly required to stand; walk; use hands to finger, handle, or feel; reach with hands and arms; stoop, kneel, crouch, crawl, talk, hear and smell.

The employee is occasionally required to sit.

Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception and ability to adjust focus.

Requires frequent heavy lifting, walking, bending and reaching.

Potential exposure to high level stimuli, e.g.- multiple simultaneous interactions, telephones, equipment noise.

Occasionally exposed to communicable diseases, unpleasant sights, and odor.

May include working some evening and/or nightshifts, weekends, and holidays.

Occasionally the employee may be exposed to the community environment (e.g.- going to clinics) and the presenting weather.

RESPONSIBILITIES

1. Include the following. Other duties may be assigned.

a) The escort is required to arrive at the Center a half an hour prior to resident scheduled appointment time.

b) Escort must be in full uniform with identification badge on.

c) Escort reports to unit nurse to receive instructions regarding resident.

d) Escort checks that resident is ready for travel

Is washed, dressed, groomed, has seasonal outer garment, as need

Has identification bracelet in place

Was recently toileted (and takes necessary items/ equipment, e.g. diaper/ wash clothes, with them).

If appointment may take a long-time, request bagged meal and drink ( for resident and escort)

e) Escort must remain with the resident and does not leave him/ her unless the consultant/ or resident requests privacy (the health care professional will then supervise resident’s care until examination is complete)

f) Escort takes any medical paperwork and clinic/ consult forms to the appointment and will ask for them to be completed (ensures findings, recommendations and treatments have follow up-makes a copy for DNS and clinic scheduler).

g) The escort requests the name and phone number for the ambulette service being used.

h) The escort and resident wait for ambulette pick up in the lobby.

i) The escort provides for the needs of the resident while out of facility.

j) If the resident is admitted to a hospital, the escort takes taxi/ or car service back to the facility (receipt is submitted to the nursing supervisor for reimbursement.)

NOTE- There is no reimbursement without a receipt.

k) Upon return, the escort brings the resident back to the unit and makes him/ her comfortable (offers water, juice and toilet, as required)

l) The escort gives the unit nurse/ -nursing supervisor a report of the trip and submits paper work. (The nurse reviews the paper work and calls the physician if immediate orders are required, otherwise the consult/ clinic report is included for physician review on his/ her visit. The nurse places the resident on the 24-hour report until the physician re-assesses the resident.)

m) The escort makes a copy of the completed consultation for the DNS and clinic scheduler.

n) The nurse will write a nurse’s note regarding resident’s condition and vital signs upon his/ her return.

QUALIFICATIONS

1. VACCINATED  ( COVID-19)

2. Must not have a record in regard to abuse, neglect or mistreatment (registry of 2 neighboring states, criminal background checks and personal references)

3. High school diploma or general education degree (GED)

4. At least six (6) months related experience and/or training; or equivalent combination of education and experience

5. Ability to read and comprehend simple instructions, short correspondence, and memos.

6. Ability to write simple correspondence.

7. Ability to effectively present information in one-on-one and small group situations to residents, visitors or other employees of the organization.

8. Ability to add and subtract two digit numbers

9. Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form.

10. Ability to deal with problems involving several concrete variables in standardized form.

11. Must exhibit a caring spirit/ attitude for the elderly/ geriatric population
