Four Pillars Recovery is growing, and we're looking for an exceptional Client Success Coordinator to join our team in Pinetop, Arizona. As a 17-bed medically supervised detoxification facility, we provide compassionate, evidence-based care to adults experiencing acute withdrawal and co-occurring behavioral health conditions. Every day, our team has the privilege of helping individuals through one of the most critical moments of their lives, offering not only expert medical care, but hope, dignity, and a foundation for lasting recovery.

OVERVIEW

The Client Success Coordinator serves as the central coordinator of each patient's treatment experience from admission through discharge, and beyond. Working collaboratively within the interdisciplinary team, community agencies, and family members, you will ensure every patient receives seamless, coordinated care. Success in this role requires excellent interpersonal communication, organization and time management, and the ability to thrive in a fast-paced, multi-facetted professional environment.

SUMMARY OF POSITION

As a Client Success Coordinator you will be tasked with managing each patient's treatment experience, including coordinating pre-admission and admission activities, assessing patient needs, coordinating services, facilitating the development of treatment and discharge planning, and monitoring treatment progress and communicating with all treatment team members, advocating for the patient during their stay and throughout the continuum of care.

ESSENTIAL JOB RESPONSIBILITIES

Serving as the primary advocate for assigned patients throughout their treatment experience, the Client Success Coordinator is responsible for the following:

  • Coordinate patient admissions, completing comprehensive intake procedures, ensuring all admission documentation, consents, and registration requirements are completed.
  • Coordinate all initial medical and clinical assessments, ensuring completion.
  • Assist the Utilization Review team with obtaining prior authorization and concurrent reviews.
  • Collaborate with multidisciplinary team to develop and implement individual treatment plans, participating in ongoing reassessments and modify care plans as patient needs evolve, communicating patient progress, concerns, and recommendations to treatment team.
  • Develop comprehensive discharge and aftercare plans, coordinating referrals to appropriate resources to ensure a smooth transition between levels of care.
  • Collaborate with families and outside agencies to support positive patient outcomes.
    Ensure all case management activities comply with company policies, regulatory standards, and best practices.

WHAT SUCCESS LOOKS LIKE

MINIMUM QUALIFICATIONS

  • 2+ years of verifiable experience working within healthcare or as an administrative assistant
  • Valid Driver's License, CPR, First Aid, and AZDPS Level I Fingerprint Clearance Card
  • Demonstrated proficiency with a variety software systems including Microsoft Office, CRMs, and EMRs
  • Customer service focused with strong interpersonal communication skills and ability to positively collaborate within a multidisciplinary team
  • Excellent organizational, task, and time-management skills
