Job description

Description The Vocational Services Case Manager is responsible to provide, coordinate and monitor client services in accordance with local, state and federal standards. The Vocational Services Case Manager will work with state agencies, funding sources, care managers, area businesses, clients and family members of clients to ensure quality of services. The Vocational Services Case Manager must understand, support and work within the values, mission, and vision of the agency.

This position is located at New View Industries in Gillett, WI. Examples of Duties Assess client needs, develop case plans, safety plans, coordinate services, advocate, monitor and document progress through regular contacts in a variety of settings, including but not limited to: homes, schools, work sites, police departments, and other service agencies. Provide intake services, i.e

gather information, assess safety, make appropriate referrals to client programming, and determine program eligibility. Provide outreach and/or crisis intervention to individuals and families. When appropriate, develop crisis and/or behavioral support plans to help support client's needs.

Participate in the development, implementation and evaluation of programs. Consult and collaborate with agency staff and outside resources to assure ongoing communication and services coordination. Develop resources and programming to meet individual and family needs.

Participate in and/or facilitate agency and community meetings, committees, and program evaluation activities. Complete all required documentation in a timely manner. Maintain client records to include paperwork required by specific programs, and information necessary to develop individual progress reports.

Track and monitor individual clients. Compile records and prepare reports. Update reports as required or upon changes to client condition.

Monitor expenditures and budgets associated with client case plans and services. Market and promote Department services with area agencies, funding sources, schools. Monitor client authorizations from funding sources.

Work with client care managers in resolving issues regarding authorizations. Maintain confidentiality of information regarding individuals and families in accordance to agency, state, and federal regulations. Participate in program staff development to enhance professional skills and knowledge through training and ongoing performance evaluations.

Attend and participate in staff, agency and other required meetings, and record reviews. Provide advocacy for clients. Keep up to date with Wisconsin States Statutes, standards, and program regulations as they relate to agency services.

Coordinate or provide transportation for individuals of families to access community resources. Provide employment related training, job counseling, job coaching, job development, benefits analysis, and arrange transportation for supported employment clients. Comply with agency and county policies and procedures.

Comply with State and Federal guidelines and requirements and specific program policies. Mandate reporter for child abuse and neglect. Assists in the writing of grants and funding proposals to support existing services and develop new programs and community resources.

May be required to perform on-call duties during business hours and after hours. May be required to work outside of normal business hours. Prepare for and respond to community emergencies including, but not limited to, natural and manmade disasters.

Perform all other duties as assigned. Typical Qualifications Bachelor's degree (BA) from four-year accredited college or university in human service or business related field. Experience and/or field placements in program area preferred.

Able to use tact and diplomacy in dealing with fellow employees, clients and their families, legal guardians, care managers, general public, and other public and private agencies. Knowledge of public benefits and ability to assist clients to obtain/maintain these benefits. Able to work with limited daily supervision and make independent decisions regarding planning, scheduling and in completing case assignments in order of priority and necessity.

Excellent organizational skills and the ability to influence the actions of others. Self-direction, and self-motivation are required. Capable of working flexible hours to meet client/family needs.

Knowledge of the principles and practices used in working with people who have varying abilities and experiences. Proficient computer utilization skills.
