Responsibilities
  • Conduct comprehensive assessments to determine client care needs and preferences.
  • Provide expert advice and recommend appropriate home care solutions tailored to each family.
  • Develop and maintain strong relationships with clients, caregivers, and healthcare professionals.
  • Coordinate care plans and ensure seamless communication between all parties involved.
  • Stay informed of industry trends, resources, and regulations to best serve clients.
  • Manage administrative tasks related to care consulting and client records.
  • Lead and mentor other care consultants as needed.
Requirements
  • Proven experience in care consulting, social work, nursing, or related healthcare roles.
  • Strong interpersonal and communication skills with a compassionate approach.
