Job description

HomeCare Connections is a Personal Service Agency licensed through Indiana Department of Health to provide (1) attendant care services; (2) homemaker services that assist with or perform household tasks, including housekeeping, shopping, laundry, meal planning and preparation, and cleaning; and (3) companion services that provide fellowship, care, and protection for a client, including transportation to and from client's residence.
Client Care Coordinator/Caregiver Coach will be responsible for overseeing and coordinating Client's care. Your expertise in Client care, communication, and coordination will be crucial in ensuring the delivery of high-quality and personalized care to our clients. You will serve as a liaison between Clients, their families, and our team of healthcare professionals to ensure a seamless and positive care experience.
POSITION REQUIREMENTS - JOB KNOWLEDGE, PERSONAL ABILITIES:
  1. Coordinate and manage the care of homecare Clients, ensuring their needs are met and their care plans are followed.
  2. Provides education to families, friends, and the community on the value of caregivers and available resources to support them. The Family Caregiver Coach assists the caregiver with identifying needs and helps facilitate goal setting and development of person-centered plans.
  3. Conduct initial assessments of Clients' medical, physical, and emotional needs to develop personalized care plans.
  4. Collaborate with Clients, their families, and healthcare professionals to establish goals and expectations for care.
  5. Facilitate support group sessions that offer caregiver education, information about community resources and/or emotional support and networking with other caregivers.
  6. Schedule and coordinate home visits, medical appointments, and other necessary services for Clients.
  7. Communicate and coordinate with healthcare providers, including physicians, nurses, therapists, and social workers, to ensure continuity of care.
  8. Monitor and evaluate Clients' progress and adjust care plans as needed to meet changing needs.
  9. Provide education and support to Clients and their families regarding their care plans, medications, and available resources.
  10. Maintain accurate and up-to-date Client records, including medical histories, care plans, progress notes and periodic customer satisfaction surveys.
  11. Partner with caregiver to develop a customized plan that may include goal setting, problem-solving, and ongoing support to reach established goals
  12. Ensure compliance with all relevant regulations, policies, and procedures related to Client care and confidentiality.
  13. Provide caregivers education and skills development including self-advocacy, coping, self-care, and disease self-management skills.
  14. Stay updated on industry best practices and emerging trends in homecare and Client care coordination.

QUALIFICATION
  1. At least 3 years' experience in nursing, healthcare administration, or a related field (preferred).
  2. Proven experience as a Client Care Coordinator or in a similar role, preferably in the homecare or healthcare industry.
  3. Strong knowledge of Client care principles, practices, and regulations.
  4. Excellent communication and interpersonal skills, with the ability to effectively interact with Clients, families, and healthcare professionals.
  5. Compassionate and empathetic approach to Client care, with a focus on providing personalized and Client-centered services.
  6. Strong organizational and time management skills, with the ability to prioritize tasks.
  7. Knowledge of homecare regulations, policies, and procedures is an asset.
  8. Ability to work independently and as part of a team, demonstrating flexibility and adaptability in a fast-paced environment.
  9. Strong problem-solving and decision-making skills, with the ability to anticipate and address potential issues.
