Description of the role:

The Client Account Manager at Preferred Care at Home of Champlain Valley offers an exciting opportunity to work from the comfort of your own home in Vermont. As a vital member of our team, you will play a crucial role in onboarding, coordinating, and managing care services for our clients.  This is a base + commission role where you will be managing a number of client cases to ensure successful delivery of care for a diverse client base.  You will be the primary contact for our clients and client's family members throughout the time that care is provided. 

Responsibilities:

  • Coordinate care services, ensuring the needs of all clients are met
  • Supporting the creation and maintaining client care plans
  • Use software scheduling program to efficiently schedule the complex week to week calendar for caregivers
  • Communicate effectively with clients and caregivers to ensure smooth and effective care management
  • Maintain accurate and up-to-date client records and documentation
  • Collaborate with other team members to ensure seamless delivery of care services
  • Handle any client concerns or issues with professionalism and empathy
  • Answer calls and texts promptly and courteously in a professional manner. 
  • Ensure timely and professional communication with clients and colleagues while working remotely from home.
