Those seeking to be a Classroom Volunteer/Volunteer Coach/Chaperone will need to complete and submit the following:
  1. Complete this Volunteer Application  (attach identification, TB test or TB Risk Assessment form and mandated reporter training)        

  2. Mandated Reporter Training (2 hours)- CDSS Child Abuse Mandated Reporter Training; submit certificate of completion with packet

  3. All above completed documents can be attached to your application in Frontline.
