The Charge Registered Nurse (RN) position is responsible for the day to day coordination of patient care within the scope of the services of the after-hours team. The Charge RN provides leadership and direction to the clinical staff for their designated shift in collaboration with management to promote high quality, timely, safe patient care. Serves as staff liaison to the On Call providers, management, and patient care team.

Essential duties and responsibilities for a Charge Registered Nurse (RN):

As a leader on their shift, the Charge RN sets the tone for staff by demonstrating a consistently positive attitude and proactive problem solving.

Keeps managers informed of concerns from staff on their designated shift and participates in collaborative problem solving to promote teamwork and job satisfaction.

Must be able to competently perform all functions of the office RN/LPN Coordinator duties, inclusive of all platforms and documentation requirements. Assists to process HCHB workflow timely and accurately.

Demonstrates ability to identify all physical, psychosocial, environmental, safety, emotional and spiritual needs related to the terminal illness to promote the patient's well-being, comfort, and dignity throughout the dying process and documents findings on all appropriate forms and on the plan of care.

Responsible for triaging patient assignments on the designated shift, considering staffing and patient acuity to promote high quality, timely scheduling, and safe patient care.

Demonstrates clinical competency and ability to mentor newly hired and existing staff and participates in the onboarding of new after-hours staff.

Executes patient/family/caregiver teaching in all given situations including disease progression and etiologies of common symptoms (i.e. pain, anxiety, delirium, SOB, edema, nausea & vomiting etc.).

Acts as first responder on their shift relative to any patient or caregiver/family complaint or care concern and keeps management informed as issues arise.

Anticipates patient/family needs or problems and makes recommendations for changes in the POC in collaboration with the IDT and attending MD.

Adheres to requirements of both CHPC and Hospice Buffalo policies and procedures that are both patient focused (clinical) and organizational focused (administrative); Adheres to HIPPA regulations and policies.

Participates in the after-hours shifts as defined and holiday schedule rotation.

Qualifications for a Charge Registered Nurse (RN):

Education/Certification

Graduate of an accredited school of nursing with current RN licensure in the State of New York.

Certification in Hospice and Palliative Care Nursing preferred.

Valid Driver's License, free of serious violations, and vehicle required for use when on duty or on-call.

Experience

Minimum of (2) years of nursing practice as a professional nurse is required.

Hospice or palliative care experience preferred.

Charge Nurse experience preferred.
