Job description

Principal Function:ย Under the direction of the Nurse Manager and the Maternal Health Nurse Care Coordinator, the Maternal and Child Health Navigator/Community Health Worker will work with underserved communities to provide resources for healthcare and maternal child services. Duties are aimed at building relationships with families. Community Health Worker needs to build trust and have good communication skills. They will provide family outreach, screenings, navigation, referral follow up and connection to resources as needed for maternal and child health. Collaborate with healthcare providers and community resources to connect individuals with the needed services, medical care, social services, and emergency assistance. Gather and relay data back to the healthcare organization.

Essential Duties and Responsibilities:ย Duties include, but not limited to:

  • Connect with the community and build trust with the clients,
  • Good communications skills.
  • Provide information on resources availability
  • Promote maternal and child health and wellness.
  • Navigate the healthcare system.
  • Access social determinants of health and make community connections as needed. Such as transportation, food, housing, insurance, language, and other barriers.
  • Connect families to WIC and community services.
  • Track missed appointments and proactively contact patients to reschedule care.
  • Provider culturally sensitive education on prenatal care, infant safety, safe sleep practices, immunizations, developmental milestones, and nutrition during office visits and phone calls.
  • Assist with appropriate data collection and tracking as directed. This may include but not limited to pregnancy through postpartum and infant care; tracking of abnormal results and outgoing referrals.
  • Promote local mental health and substance use services and raise community awareness.

No supervisory responsibilities.

Education and/or Experience:ย High school diploma or general education degree (GED), or one-year related experience and/or training; or equivalent combination of education and experience. A Community Health Worker certification is required.
