Position Overview
The Patient Care Associate (PCA) plays a key role in supporting patients enrolled in
ChartSpan's care management programs. This role focuses on delivering compassionate,
efficient, and accurate assistance through inbound and outbound calls, helping patients
navigate their care, manage appointments, understand instructions, and access needed
resources. The PCA works closely with the clinical team and dedicated nurses to ensure
seamless coordination and a positive patient experience.
Requirements
Responsibilities May Include:
  • Communicates resources and services available to patients through the
  • continuum of care.
  • Maintain patient chart compliance through proper documentation and updates of:
  • medical history, medication, immunizations, allergies, surgical history, and family
  • history.
  • Assist patients with understanding care instructions, navigating services, and
  • addressing non-clinical questions
  • Answer inbound patient calls with professionalism, empathy, and clarity
  • Conduct outbound calls for follow-ups, appointment reminders, wellness checks,
  • or care coordination
  • Maintain patient confidentiality and adhere to HIPAA standards
  • Follow call scripts, workflows, and escalation pathways
  • Meet performance metrics such as call quality, accuracy, and responsiveness

Requirements:
  • Active and unencumbered certification from an accredited regulatory body such as AAMA, NHT, NHA, NCCT, Pharmacy Technician registries, and State Nurse Aide registries.

Accepted certifications include:
  • Certified Nursing Assistant (CNA)
  • Certified Clinical Medical Assistant (CCMA)
  • Certified Pharmacy Technician(CPhT)
  • Patient Care Technician (CPCT/A)
  • Emergency Medical Technician (EMT)
  • Licensed Respiratory Therapist (RT or CRT)

Salary Description
