Job Overview

Ebenezer is looking for a Adult Day Program Assistant to join our team! This position encourage socialization and assists in planning and implementation of programming needs of individual participants and groups and assist with personal care duties. Responsible for ongoing charting and case management for our Skilled Nursing Facility in Bloomington, MN. 

This schedule includes;

  • 80 hours per two weeks
  • Day shift 
  • 8:00am-4:30pm
  • No weekends 

Martin Luther Care Center, part of the Martin Luther Senior Living Campus is located in East Bloomington, near woodlands/wetlands and along the Minnesota River Bluffs. Managed by Ebenezer, we offer 137 units between our Skilled Nursing, Memory Care and Transitional Care. We have an outstanding team of tenured staff. The only thing missing is YOU! Martin Luther Care Center was named Top Nursing Home and Transitional Care Center by US News and World Reports the past two years.

Responsibilities

  • Assist the Director in the efficient operation of the program:
    • Conducts tours.
    • Meets with families and assists in filling out enrollment paperwork.
    • Communicates with Case Managers and families as needed.
    • Oversees charting of Program Assistants to ensure accuracy and timeliness.
    • Assist with setting up care plans.
    • Conducts quarterly/annual assessments as needed/directed.
    • Assists with office duties (attendance, billing).
    • Functions as Lead staff in absence of Director.
  • Responsible for individual case management:
    • Set up participants’ charts per licensing requirements.
    • Ensure that all the forms are completed by family and staff and updated as necessary.
    • Maintain monthly charting, quarterly care plans and health updates.
    • Document monthly health education, fire drills, blood pressures and weights for each participant.
  • Ability to provide direct care for the Adult Day Program participants:
    • Responsible for preparation, implementation and clean up of daily programs i.e. crafts, woodworking, games, music, etc.
    • Responsible for providing snacks and noon meal to participants. Set up, cutting up food, feeding participants as necessary and clean up.
    • Assist staff in developing monthly calendar.
    • Responsible for daily menu planning and ordering of meals and snack supplies.
    • Performs routine personal care and grooming at the center (including baths).
    • Responsible for assisting identified participants with all toileting needs.
    • Promote and encourage individual care.
    • Provide individual and family counseling for participants as needed.
    • Communicate with staff the participants needs and individual plan of care.
    • Observe, assess and intervene where needed with participants health concerns.
    • Reports any problems, progress and observations to the Director.


Required Qualifications

  • CPR certification and Basic First Aide Training

Preferred Qualifications

  • A.A./A.S.
  • less than 1 year
