Overview
Position located in Manchster, NH
Provides assessment, counseling, information and referral, advocacy services, and other resource assistance to patients directly or through consultation.
Job Location: TMA01
Responsibilities
  • Provides complete psychosocial assessments of patients and families.
  • Provides counseling to individuals and families to help solve social and emotional problems.
  • Coordinates community resources to address/meet identified needs.
  • Assists families in adapting to difficult and complicated dispositions involving complex/catastrophic care needs, multiple agencies/providers and linkage to resources outside the community.
  • Provides consultation, assessment, referral and coordination in cases of suspected child or dependent adult abuse or neglect, or other forms of domestic violence.
  • Organizes support groups.
  • Documents objective, concise and pertinent clinical data in the medical record.
  • Attends multidisciplinary clinics with the CF team
  • Conducts regular screening for depression and anxiety
  • Builds familiarity with and helps patients access CF-related resources
  • Attends national and regional CF conferences yearly along with other team members
  • Serve as a Mental Health Coordinator as defined by the CF foundation, helping patients locate mental health providers including therapists and psychiatrists for ongoing mental health needs

  • Qualifications
    • Master's Degree in Social Work (MSW) required.
    • Social work experience in an acute, home care, or ambulatory setting preferred.
    • Strong organizational skills, verbal and written communications skills.
    • Sound decision making and judgment.
    • Time management skills.
    • Negotiation skills.
    • Proven ability to deal with conflict resolution.
    • Comfortable with basic computer skills.
