Salary: $126,000 - $132,000 annually
MAKE A DIFFERENCE IN THE LIVES OF OLDER ADULTS

As a Care Delivery Manager, you will play a critical leadership role in overseeing care delivery, supporting interdisciplinary teams, and helping older adults remain safely and independently in their communities.
This is an opportunity to lead with purpose while making a meaningful impact on participant outcomes and quality of life.
WHAT YOU'LL DO
  • Oversee the development, implementation, and ongoing monitoring of individualized care plans
  • Coordinate and facilitate Interdisciplinary Team (IDT) meetings and ensure timely coordination of participant services
  • Monitor quality outcomes, regulatory compliance, clinical documentation, and operational performance metrics
  • Collaborate with physicians, nurses, social workers, therapists, and community partners to support integrated, participant-centered care and seamless care delivery
  • Manage staffing, scheduling, and resource allocation to support operational effectiveness
  • Provide leadership, coaching, mentorship, and professional development to team members while promoting a culture of accountability, collaboration, and excellence
  • Participate in leadership coverage and support participant care needs as required

WHAT WE'RE LOOKING FOR
Required:
  • Active and unrestricted Registered Nurse (RN) license in New York State
  • Previous leadership or management experience in healthcare, home care, managed care, PACE, long-term care, or care management environments
  • Experience leading teams, managing performance, and driving quality outcomes
  • Strong communication, problem-solving, and organizational skills

Preferred:
  • BSN
  • Experience working with frail or elderly populations
  • Bilingual skills
  • Case Management Certification
