Job description

Administrative Care Manager

Lead Operations. Drive Excellence. Improve Participant Care.

CenterLight Healthcare PACE is seeking an experienced Administrative Care Manager to coordinate day-to-day interdisciplinary operations, oversee participant care processes, and drive operational excellence across the PACE center. This role partners closely with clinical and operational leaders to ensure high-quality, compliant, and participant-centered care.

What You'll Do

  • Coordinate the day-to-day operations of the interdisciplinary team and support seamless participant care across the continuum.
  • Partner with site leadership and department managers to optimize workflows, operational performance, and participant outcomes.
  • Monitor operational metrics and identify opportunities for quality improvement and enhanced care delivery.
  • Support the development and execution of individualized participant care plans.
  • Coordinate staff education, in-services, and interdisciplinary collaboration.
  • Lead and participate in quality assurance, regulatory readiness, and compliance initiatives.
  • Collaborate with Shared Services and operational partners to improve processes and implement best practices.
  • Monitor participant satisfaction and voluntary disenrollment trends.
  • Serve as the LHCSA Administrator and participate in the Administrator On-Call rotation.

What We're Looking For

Required Qualifications

  • Bachelor's degree in Business Administration, Healthcare Administration, or related field.
  • Minimum of 3 years of recent healthcare operations experience.
  • At least 1 year of experience working with older adults in PACE, MLTC, or CHHA.
  • Strong organizational, communication, problem-solving, and relationship-building skills.

Preferred Qualifications

  • Master's degree preferred.
  • Bilingual preferred.
  • Experience with healthcare quality improvement, regulatory compliance, and interdisciplinary care coordination.
