Hospice RN Case Manager $85,000$100,000 | MonFri | ~15-Patient Caseload

Location: Cherry Hill NJ
Schedule: MondayFriday | 8:00 AM5:00 PM
On-Call: Rotating | Limited occasional evenings/weekends
Caseload: Approximately 15 patients (subject to change based on census and acuity)

Your Role

As an RN Case Manager, you are the clinical lead for your assigned patients supported by a strong interdisciplinary team.

You will:

Conduct comprehensive nursing assessments (with Admissions Nurse support)
Develop and manage individualized plans of care
Collaborate with physicians, LPNs, social workers, chaplains & aides
Educate families on symptom management and disease progression
Participate in IDT meetings
Maintain timely, compliant documentation
