Position Summary:
The Admissions Coordinator is responsible for managing the referral and admissions process, maintaining census goals, and serving as a liaison between the facility, hospitals, families, and the community.
Key Responsibilities:
  • Manage incoming referrals and coordinate the admissions process from start to finish
  • Communicate with hospitals, case managers, physicians, and families
