Your Opportunity:

If you are interested in working within an interprofessional team playing a key role in linking the needs of Home Care clients with appropriate health and community services then this position will be of interest to you. Based on the Continuing Care Case Management Competency Framework you demonstrate ability in the areas of personal and professional accountability, systems thinking, facilitating creative thinking, communicating and building relationships. Using a person-centered approach you will develop a care plan supporting the client/family to identify needs, focus on strengths, set achievable personal health and self-care goals and provide direct nursing care. In collaboration with the client/family you will ensure services are provided and modified on an ongoing basis, based on client's response to interventions, incorporating stewardship into service authorization. This position is with the Self Managed Care Team which is a specialty Team within Integrated Home Care. This Team looks after the needs of our clients who choose to hire their own Caregivers.

Description:

As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being. In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance. You will provide safe, quality patient and family centered care while reflecting the shared vision and values. PLEASE NOTE: Provisional Permit holders will be paid $40.76 to $55.79 per hour. Upon obtaining a full practice permit, the rate of pay will be adjusted to the applicable Registered Nurse rate in accordance with UNA Article 25.02(b)(i) or (ii), as applicable.

Required Qualifications:

Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP) certification. Additional Required Qualifications:

Diploma in Nursing required. Graduation from a recognized Nursing education program. Active registration and practice permit with the College and Association of Registered Nurses of Alberta required (CARNA). Basic Cardiac Life Support -Health Care Provider (BCLS-HCP) required. Minimum of two years current nursing experience. Intermediate level of computer skills; excellent organizational and time management skills with strong interpersonal skills. Ability to perform all duties in accordance with related legislation. Note: This position requires a vehicle for business use. Possession of a valid driver's license and access to a motor vehicle in good repair for work related travel is required.

Preferred Qualifications:

Baccalaureate degree in Nursing preferred. Minimum one year experience in a community based setting preferred. Assets include: recent experience working with acute clients and/or long term clients with a focus on chronic disease management; Home Care experience preferred.
