Job description

Gulfstream Staffing has partnered with a medicare-certified Home Care organization that is experiencing a rapid growth phase. We are currently seeking candidates for a full-time RN Case Manager opportunity in Vero Beach, FL.
The Case Manager is required to primarily work in office but may also be required to perform field case management with responsibilities for assessment, and provision of skilled care to the patient. The Case Manager role requires communication with referral sources, coordination of the patient admission with other members of the care team and must have strong case management experience and a working knowledge of home care.
Position: RN Case Manager
Location: Vero Beach, FL
RN Case Manager Duties:
-The Case Manager is required to perform field case management with responsibilities for assessment, and provision of skilled care to the patient
-Perform case management, and oversight and coordination of care for all skilled disciplines
-Provide nursing care and treatment of home care patients in accordance with the plan of treatment and approved authorizations
-RN will supervise all non-skilled caregivers
RN Case Manager Qualifications:
-Current RN Florida Professional license
-Minimum of one year experience with home care case management
-Must have excellent time management skills to prioritize and organize workload
