At Senior Solutions (Area Agency on Aging in Southeastern Vermont), we are seeking a Case Manager to work with older adults and/or people with disabilities in the White River Junction/Windsor and the surrounding areas using a person-centered planning approach and a deep understanding of state and federal benefits. Strong organizational, communication, and computer skills are required.
Qualifications:
Bachelor's degree in human services and prior case management experience preferred.
A combination of relevant education and experience may be substituted for a degree.
