Contract | Travel | 13 Weeks | Days | Saturday - Monday, 7:00 AM - 7:00 PM
Key Responsibilities
  • Perform hospital-based case management functions including assessment, planning, and coordination of patient care
  • Facilitate discharge planning and transitions of care to ensure appropriate post-acute placement and resource utilization
  • Collaborate with interdisciplinary care teams including physicians, social workers, and ancillary staff to support patient needs
  • Monitor patient progress and adjust care plans to promote quality outcomes and efficient length of stay
  • Ensure documentation of case management activities is accurate, timely, and compliant with facility standards
Required Qualifications
  • Active Registered Nurse (RN) license valid in the state of Texas or compact licensure eligibility
  • Minimum of 1 year of experience in a case management or related clinical nursing role
  • Experience in a hospital-based setting
  • Ability to work a weekend-focused schedule (Saturday through Monday, 12-hour day shifts)
Preferred Experience / Skills
  • Prior travel nursing experience in a case management specialty
