Your Role
As a key member of our interdisciplinary care team, you will work under the supervision of a Clinical Manager to coordinate and deliver high-quality, patient-centered care. You'll play a vital role in ensuring comfort, dignity, and quality of life for those we serve.
Key Responsibilities
  • Perform ongoing, comprehensive assessments of patients and families
  • Develop and oversee individualized care plans in collaboration with the care team
  • Lead and participate in interdisciplinary case conferences
  • Coordinate care with physicians, hospice staff, volunteers, and community partners
  • Advocate for patients and families, addressing clinical, emotional, and psychosocial needs

The Pay Range listed is the anticipated hiring range of this position. Precise pay rates are determined through consideration of one or more of the following: education, training, relevant experience and work location. There are some cases in which the pay rate could exceed the range listed on this job post. Care Dimensions also offers a comprehensive benefits package - including employer-sponsored health insurance, paid time-off, tuition reimbursement, student loan paydown, supplemental insurance options and more - which adds to the company's generous total rewards package.

$47-$56 USD
