Job Description Summary:

Supervises select care management functions and daily operations.
Demonstrates excellence as a case manager and serves as role model for the department.
Act as a mentor, advisor, and educator to associates at their care site or ambulatory team, including onboarding new associates, as appropriate.
Triage urgent needs, assist with prioritizing work in accordance with EWCM needs.
Takes on a front-line assignment when indicated.
The RNCM Clinical lead will maintain expertise as RN Case Manager in clinical assessment, care planning, interventions, education, and evaluation.
Maintain competencies associated with all care management roles.
This position reports to the EWCM manager or director and may assume the role of the manager in their absence.
Participate in emerging leadership courses as available.

Responsibilities And Duties:

Demonstrates excellence as a case manager and serves as role model for the department. Participates in all meetings specific to job responsibilities (i.e., multidisciplinary rounds, team huddles, and enterprise-wide town hall meetings.) Monitors performance of staff and provides coaching as required. Assures the 90-day onboarding for associates meets the required competencies. Administrative responsibilities for scheduling staff, daily assigning of cases, caseload balancing and assisting the manager (s) with direct supervision of the care management staff and the day-to-day operations/activities of care management. Additionally, the Care Management Clinical Lead will maintain competencies associated /assigned in the Care Manager role (s) and processes. The lead may take caseloads as assigned by manager. Provides input regarding associate performance. Creates work environment where employees can express opinions openly.
Ensures that employees understand their responsibilities. Provides structure, direction, and feedback to staff. Holds staff accountable for performance. Identifies and develops associates who possess leadership qualities. Demonstrates high standards of ethics and fairness while adhering to company policies. Communicates and inspires employees to follow the system mission, vision, and values. Identifies and is open to process improvements. Willing to work hours necessary to accomplish job duties. Arranges for backup when absent. Thinks beyond personal and departmental objectives when making decisions and performing work. Maintains network of industry associates.
Stays current with best practices and strives to implement them. Leads by example. Assures department has necessary resources including appropriate staffing levels. Participates in interviewing and hiring employees for areas of responsibility. Successfully and quickly adapts to changes in assignments and role within the organization. Supports and encourages workforce diversity. Requires little or no direction in performing day-to-day responsibilities. Remains calm in times of crisis. Demonstrates active engagement in committees, team huddles, and reports out to leadership on progress towards departmental and system wide goals. Evidence of documented progress toward goals (strategic, departmental, and personal).
