  • Champion patient-centered care by designing integrated care plans that align medical necessity with the unique values and preferences of patients and their families.

  • Navigate complex psychosocial barriers by assessing and treating needs related to crisis intervention, substance abuse, and legal challenges to ensure holistic recovery.

  • Advocate for essential resources by identifying and removing systemic or financial obstacles that prevent patients from accessing the care they deserve.

  • Orchestrate seamless transitions through constant collaboration with the care team, ensuring the health plan remains effective and responsive as the patient's needs evolve.

  • Guide post-discharge success by providing expert recommendations on complex levels of care and documenting the clinical journey to ensure high-quality continuity of care.

What minimum qualifications you'll need

Licensure / Certification / Registration:

One or more of the following required:

  • Licensed Clinical Counselor credentialed from the Wisconsin Board of Marriage & Family Therapy, Professional Counseling & Social Work obtained prior to hire date or job transfer date.
  • Social Worker credentialed from the Wisconsin Board of Marriage & Family Therapy, Professional Counseling & Social Work obtained prior to hire date or job transfer date.
  • Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred .

Education:

  • High School diploma equivalency with 2 years of cumulative experience OR Associate'sdegree/Technical degree OR 4 years of applicable cumulative job specific experience required.
