Schedule: Full time Tuesday - Saturday, 8:00AM-4:30PM.

Coordinates Medicare-related communication by delivering and documenting Important Message (IM) and Code 44 letters, obtaining required signatures, and supporting appeals processes. Monitors voicemails and ensures timely follow-up.

Supports discharge planning by coordinating with vendors, identifying appropriate post-care services (e.g., home health, nursing facilities, hospice), verifying benefits, and communicating updates to care teams.

Maintains department operations by updating discharge-related systems, compiling reports and statistics, managing supplies, and providing clerical support such as meeting minutes and general administrative tasks.
