RN Case Manager - Long-Term Acute Care
Job Description
The RN Case Manager coordinates management of patient care, ensuring optimum utilization of resources, service delivery, and compliance with external review agencies. This role provides ongoing support and expertise through comprehensive assessment, care planning, plan implementation, and overall evaluation of individual patient needs. The RN Case Manager aims to enhance the quality of patient management and satisfaction while promoting continuity of care and cost-effectiveness through the integration of case management, utilization review, and discharge planning functions.
Responsibilities
  • Comprehensive assessment of patient needs and care planning.
  • Plan implementation and overall evaluation of individual patient care.
  • Facilitate resource utilization and discharge planning.
  • Serve as a patient advocate by addressing holistic needs.
  • Enhance coordination of care across acute settings.
Required Experience / Certifications / Licensure
  • Must have a current Illinois RN license.
  • Minimum of 2 years recent RN Case Management experience, preferably in Long-Term Acute Care (LTAC) or Short-Term Acute Care Hospital (STACH) settings.
  • Experience with MCG, discharge planning, and strong interdisciplinary team (IDT) collaboration.
  • BLS certification required through the American Heart Association.
Schedule
This position operates on a Monday to Friday schedule from 8:00 AM to 4:30 PM.
