Case Management Analyst - Field

Location: Northern Virginia; Arlington, Annandale, Falls Church &/or Springfield VA

Aetna's LongTerm Services and Supports (LTSS) program is looking for a passionate, memberfocused Case Management Analyst to support individuals and families across the Northern Virginia region. This is a fieldbased role where you'll spend meaningful time facetoface with members-listening, advocating, and helping them navigate complex health and social needs.

What You'll Do

  • Build trusted relationships with members through inperson visits (approximately 75% travel).

  • Conduct comprehensive assessments using clinical judgment and critical thinking to understand medical, behavioral, and social needs.

  • Develop and implement individualized care plans that promote health, independence, and quality of life.

  • Collaborate closely with providers, community resources, and interdisciplinary teams to ensure coordinated, highquality care.

  • Connect members to services, supports, and resources that align with their goals.

  • Ensure care delivery meets regulatory requirements and Aetna quality standards.

  • Advocate for medical appropriateness while supporting positive member engagement and outcomes.

Required Qualifications

  • 2+ years of experience in behavioral health, social services, or a related field aligned with program focus.

  • Must reside in Northern Virginia in one of these locations; Arlington, Annandale, Falls Church or Springfield

  • Must possess reliable transportation and be willing and able to travel up to 75% of the time. Mileage is reimbursed per our company expense reimbursement policy

Preferred Qualifications

  • Experience in case management or discharge planning.

  • Managed Care experience.

Education

  • Bachelor's degree or nonlicensed master's degree required.

  • Degree in behavioral health or human services strongly preferred.

Pay Range

The typical pay range for this role is:

$21.10 - $40.90

This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
