Join our exceptional team in Renfrew, PA, as a Case Coordinator and make a difference in the lives of our clients!

Position Details:
  • Hourly Rate: $17 - $19 per hour
  • Schedule: Full Time w/ Benefits
Essential Duties and Responsibilities:
  • Coordinate client engagement through appointments, aftercare plans, schedules, and meetings in collaboration with the treatment team.
  • Maintain professional communication with referral sources and outside providers, assessing needs and documenting contacts during admission and throughout treatment. This includes coordination for utilization review, legal matters, and employment needs for clients.
  • Provide individual case coordination sessions and services, including transportation coordination.
  • Assist in developing aftercare plans, offering referrals and contact information for aftercare providers, and addressing any needs from outside providers.
  • Support clinical services by assisting with group sessions and engaging clients weekly to enhance their participation in the program.
  • Document all interactions with clients, families, referrals, and outside providers in Carelogic, adhering to program timelines.
  • Collaborate with the client navigation team to address aftercare needs effectively.
  • Provide onsite support for Enhanced Outpatient Sessions (EOS) appointments between clients and the Client Navigation team.
  • Perform other duties as assigned.
Education, Licensure, & Experience:
  • High School Diploma or equivalent is required.
  • 1 year of experience in a drug and alcohol or mental health setting is preferred.
  • A valid Driver's License is preferred.
  • CPR/BLS certification is preferred.
