Job description

Description This is an Aging Case Aide 2 position in the Community Services Department at the Area Agency on Aging. This position is responsible for coordinating Health & Wellness programs in the Active Adult Centers and providing support services to the Senior Center Director 3. Essential Duties and Responsibilities Identifying senior population in Luzerne and Wyoming counties interested in participating in Health & Wellness programs through outreach efforts and/or referrals.

Visits Active Adult Centers to provide information regarding services for which consumers are eligible. Assists Senior Center Director 3 in providing Active Adult Center coverage when necessary. Provides detailed explanations to consumers relating to the procedural aspects of obtaining available services.

Surveys consumers who participated in the Heath & Wellness programs to evaluate outcomes and ensure programs are meeting the consumer's needs. Responsible for registration of consumers for program participation, completing class evaluations, surveys, and data entry required by the PA Department of Aging. Assists the Senior Center Director 3 and Active Adult Center Mangers to ensure programs are meeting highest tier/evidence based requirements.

Other duties as deemed necessary by the Senior Center Director 3. Required Knowledge, Skills and Abilities Knowledge of the physical, cultural and social aspects of the Active Adult Centers. Ability to obtain, evaluate and analyze information from surveys, reports and records to provide information to consumers, PA Department of Aging, and Administration.

Ability to learn about the Area Agency on Aging programs and the services of other community agencies. Ability to communicate effectively, both orally and in writing with the senior population and staff. Ability to establish and maintain positive working relationships.

Ability to express ideas clearly and persuasively to senior population. Knowledge of Microsoft Office Suite including proficiency in Microsoft Word and Excel. First Aid, CPR, and AED certification is required and will be provided.
