Licensed Practical Nurse (LPN)

Job Summary

We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our dedicated team at Shippenville Nursing and Rehab, a 25-year veteran in providing quality long-term care and rehabilitation services to the surrounding community. As a Licensed Practical Nurse (LPN), you will have the opportunity to work on a flexible schedule, providing personalized care to our residents while promoting a positive and supportive environment.

Responsibilities

• Provide direct patient care, including administering medications, taking vital signs, and performing resident assessments
• Develop and implement individualized care plans, collaborate with interdisciplinary teams, and prioritize resident-centered care
• Maintain accurate and detailed records of patient care, including medication administration, treatments, and test results
• Engage with residents, families, and caregivers to ensure effective communication and address concerns
• Participate in Quality Assurance and Performance Improvement (QAPI) initiatives, identifying areas for improvement and implementing changes to enhance resident care
