Caresource Lpn information

What is a Caresource LPN?

A CareSource LPN (Licensed Practical Nurse) provides healthcare support to patients within CareSource's managed care system. They assist with patient assessments, medication management, care coordination, and health education under the supervision of registered nurses or physicians. Their role focuses on improving patient outcomes by ensuring access to quality care, particularly for Medicaid and Medicare members. LPNs may work in clinical settings, telehealth, or case management to support CareSource’s mission of providing compassionate healthcare.

What does a Caresource LPN do?

A typical day for a CareSource LPN involves coordinating care for members, conducting telephonic or in-person health assessments, and collaborating closely with case managers, providers, and community resources. You’ll regularly review patient charts, follow up on care plans, and help members access the resources they need to maintain or improve their health. The role often balances administrative responsibilities with direct patient or member engagement. Teamwork is essential, as LPNs frequently communicate with clinical and non-clinical colleagues to address members’ needs efficiently and effectively.

What skills and qualifications are needed to be a Caresource LPN?

To thrive as a CareSource LPN, you need a practical nursing diploma, an active LPN license, and solid clinical assessment and care coordination skills. Familiarity with electronic health records (EHR) systems, remote patient monitoring tools, and managed care protocols is commonly expected. Strong communication, problem-solving, and organizational skills help LPNs excel in collaborating with patients, providers, and care teams. Mastering these competencies supports effective patient care, regulatory compliance, and smooth care transitions within a managed healthcare environment.
