Empower Health NP is seeking a dedicated and compassionate Nurse Practitioner to join our dynamic team in Rome, New York. This full-time onsite role offers an annual salary ranging from USD 150,000 to 160,000, commensurate with experience. As a Nurse Practitioner, you will play a vital role in delivering high-quality healthcare services to a diverse patient population in a hospital setting. Key Responsibilities
  • Conduct thorough health assessments and physical examinations, developing individualized care plans based on thorough analysis of patient histories and current health statuses.
  • Diagnose and manage a range of acute and chronic medical conditions, coordinating preventive care strategies and health promotion activities.
  • Collaborate closely with physicians, nurses, and healthcare professionals to formulate patient treatment plans and ensure continuity of care.
  • Engage with community organizations to develop and implement educational programs that address prevalent health issues and promote wellness.
  • Advocate for patients' rights and access to healthcare services, ensuring culturally sensitive care and respect for diverse backgrounds.
  • Maintain accurate, up-to-date medical records and documentation in compliance with healthcare regulations and organizational policies.
  • Participate in outreach initiatives and health fairs to promote wellness and preventive health screenings in the community.
  • Stay abreast of current health trends, policy changes, and evidence-based practices relevant to community health care.
Patient Population / Client Focus

Your practice will primarily focus on serving the diverse patient population in Rome, NY, including individuals from various socioeconomic and cultural backgrounds. You will be instrumental in providing care that addresses barriers to access and enhances overall community health outcomes.

Qualifications
  • Master's or Doctoral degree in Nursing from an accredited institution.
  • Certification as a Family Nurse Practitioner (FNP), Adult-Gerontology Nurse Practitioner (ANP), or Adult-Gerontology Primary Care Nurse Practitioner (AGNP) from the American Nurses Credentialing Center (ANCC) or the American Academy of Nurse Practitioners (AANP).
  • Current, unrestricted Nurse Practitioner (APRN) license in New York State is mandatory.
  • Possession of a valid DEA# and NPI#.
  • A minimum of 3 years of clinical experience, preferably within a primary care or family medicine setting.
  • Strong clinical assessment, diagnostic, and decision-making skills are essential.
  • Excellent communication and interpersonal skills, with effectiveness in engaging with diverse populations.
  • Ability to work independently while effectively collaborating as part of a healthcare team.
  • Proficient in using electronic health records (EHR) to facilitate efficient patient documentation and data management.
Preferred Experience
  • Experience in community health or public health settings is highly desirable.
  • Demonstrated ability in developing and implementing health education programs.
  • Strong understanding of public health policies and best practices related to community health.
