Empower Health NP is seeking a dedicated and experienced Nurse Practitioner to join our team in Troy, New York. This full-time position offers a competitive annual salary range of USD 150,000 to 160,000. Key Responsibilities

• Conduct thorough health assessments and screenings for patients to determine their individual health needs.
• Diagnose and manage both acute and chronic medical conditions with a focus on preventive care and health promotion.
• Collaborate with multidisciplinary teams including physicians, nurses, and support staff to develop care plans that prioritize patients’ health outcomes.
• Educate patients and their families about health management, preventive measures, and treatments.
• Document all patient interactions and maintain accurate and detailed medical records in compliance with state and federal regulations.
• Lead community outreach programs aimed at promoting health education and awareness about available healthcare services.
• Participate in health fairs and community events to deliver screenings and health education to underserved populations.
• Remain informed about the latest health trends and policies relevant to community health to better serve patients.

Patient Population / Client Focus

As a Nurse Practitioner at Empower Health NP, you will engage with a diverse patient population that includes individuals from various socioeconomic backgrounds, ages, and demographic groups. Qualifications

• A Bachelor’s degree in Nursing from an accredited institution.
• Active Nurse Practitioner (APRN) license in New York State, with current certification as a Family Nurse Practitioner (FNP), Adult-Gerontology Nurse Practitioner (ANP), or Adult-Gerontology Primary Care Nurse Practitioner (AGNP).
• Possession of a valid DEA# and NPI# is required.
• A strong foundation in clinical assessment, diagnostic skills, and proficient use of electronic health records.

Preferred Experience

• A minimum of 3 years of clinical experience in community health, family medicine, or related healthcare settings.
• Demonstrated success in fostering effective communication and building rapport with patients from diverse populations.
• Experience leading health programs or initiatives that improve patient outcomes at the community level.

Compensation and Benefits

In addition to the competitive annual salary of USD 150,000 to 160,000, Empower Health NP offers a comprehensive benefits package that includes health insurance, retirement savings plans, paid time off, and educational support for continued professional development.
