Work Environment

This position requires the Nurse Practitioner to work onsite at a hospital setting, allowing for real-time patient engagement and collaboration with multidisciplinary teams. The typical work schedule involves weekday hours from 9:00 a.m. to 6:00 p.m., with the possibility of extended hours for evening clinics until 8:00 p.m. Some weekend outreach participation may also be necessary, particularly for community health fairs and events. Overall, you can anticipate an average workweek of approximately 40 hours.

Key Responsibilities
  • Conduct thorough health assessments and screenings for patients of all ages, ensuring individualized care plans that address acute and chronic health conditions.
  • Diagnose and manage various medical conditions, emphasizing the importance of preventive care and health promotion strategies.
  • Collaborate effectively with community stakeholders and organizations to design and implement impactful health education programs focused on enhancing awareness and access to services.
  • Provide culturally sensitive care and advocate for patient rights, ensuring a comfortable and inclusive environment for all patients.
  • Coordinate patient referrals and ensure continuity of care, meticulously following up on each patient's progress and treatment plans.
  • Maintain comprehensive and accurate medical records, adhering to all regulatory standards to protect patient privacy.
  • Engage in ongoing community outreach initiatives, actively participating in health fairs that provide health screenings and educational resources.
  • Stay informed about emerging community health trends, evidence-based practices, and relevant public health policies to better serve the population.
Patient Population / Client Focus

The Nurse Practitioner will primarily engage with a diverse patient population, focusing on various demographic groups that reflect the communityโ€™s unique socioeconomic backgrounds. You will play a pivotal role in addressing health disparities, ensuring that every patient receives the attention and care they need, tailored to their specific circumstances and cultural context.

Qualifications
  • A Masterโ€™s or Doctoral degree in Nursing from an accredited institution.
  • Current certification as a Family Nurse Practitioner (FNP), Adult-Gerontology Nurse Practitioner (ANP), or Adult-Gerontology Primary Care Nurse Practitioner (AGNP) through the American Nurses Credentialing Center (ANCC) or the American Academy of Nurse Practitioners (AANP).
  • Possession of a current, unrestricted Nurse Practitioner (APRN) license in New York State.
  • DEA and NPI numbers are required to practice.
Preferred Experience
  • Minimum of 2 years of experience in primary care or family medicine preferred.
  • Demonstrated competence in clinical assessment and diagnostic skills.
  • Strong communication and interpersonal skills to effectively connect with patients and collaborate with healthcare teams.
  • Ability to work independently as well as cohesively within a multidisciplinary team.
  • Proficiency in employing electronic health record (EHR) systems to manage patient information efficiently.
  • A background in community health or public health will be advantageous to success in this role.
Compensation and Benefits

This rewarding Nurse Practitioner position offers a competitive annual salary ranging from USD 150,000 to 160,000, reflecting your skills and experience.
