Key Responsibilities
  • Conduct thorough health assessments, including physical examinations, to diagnose and manage both acute and chronic medical conditions.
  • Develop and implement individualized care plans in collaboration with patients, ensuring that all treatment goals are addressed.
  • Educate patients and their families about health management, preventive care practices, and the importance of follow-up appointments.
  • Advocate for patient's rights and help navigate the healthcare system to ensure access to necessary services.
  • Maintain precise medical records using electronic health record (EHR) systems, ensuring compliance with established healthcare regulations.
  • Engage in community outreach efforts, participating in health fairs and educational events to promote health awareness and screenings.
  • Monitor and evaluate patient progress, modifying care plans as necessary, and coordinating referrals to specialists when required.
  • Stay informed on community health trends and evidence-based guidelines to provide optimal patient care.
Patient Population / Client Focus

Your primary focus will be on serving a diverse patient base, including families and individuals with varying health needs. You will play a critical role in addressing barriers to healthcare access, ensuring culturally sensitive care while managing both preventive and acute health issues. Your position enables you to make a significant impact on health outcomes within the community.

Qualifications
  • A Master's or Doctoral degree in Nursing from an accredited institution.
  • Certification as a Family Nurse Practitioner (FNP), Adult-Gerontology Nurse Practitioner (ANP), or Adult-Gerontology Primary Care Nurse Practitioner (AGNP) from a recognized certifying body (ANCC or AANP).
  • A current, unrestricted Nurse Practitioner (APRN) license to practice in New York State.
  • DEA# and NPI# are required.
  • Exceptional clinical assessment and diagnostic skills.
  • Proficiency in electronic health record (EHR) systems.
  • Strong interpersonal skills with a proven ability to communicate effectively with patients and team members.
Preferred Experience
  • A minimum of 2 years of experience as a Nurse Practitioner, with a preference for candidates who have worked in community health or public health settings.
  • Experience in primary care or family medicine is highly desirable.
  • Demonstrated ability to work both independently and collaboratively in a multidisciplinary team environment.
Compensation and Benefits

The annual salary for this Nurse Practitioner position ranges from USD 150,000 to 160,000, commensurate with experience and qualifications. In addition to competitive pay, we offer a comprehensive benefits package that includes health insurance, retirement plans, paid time off, and a sign-on bonus of up to $10,000 to help you transition into your new role.
