Their role may involve conducting assessments, coordinating services, and offering emotional support to promote better health and quality of life.

What does a care coach do?

A typical day for a Care Coach involves working directly with clients to assess their needs, develop customized care plans, and provide ongoing support and resources. You may spend time conducting phone or virtual check-ins, collaborating with healthcare providers and social service agencies, and documenting case progress in care management systems. Care Coaches often balance independent work with team meetings to discuss complex cases and share strategies. This role is dynamic and people-centered, making strong organizational skills and adaptability especially important.
