Social Worker
Location: Falls Church, VA
Shift: Monday -Friday 3 eight hour shifts per week- 24 hours/week
GENERAL DESCRIPTION SUMMARY
The Hospice Social Worker is an essential member of the Capital Caring Health Interdisciplinary Team, providing a wide variety of services to patients/caregivers/families in accordance with the individualized plan of care. The Social Worker actively participates in coordination of all aspects of the patient's care in accordance with current professional standards and practice. The Social Worker's expertise lies in identifying and addressing the emotional and biopsychosocial needs in advanced illness at end of life.
Experience
Must have at least 2 years of Master's level Social Work experience in an interdisciplinary or health care setting.
Education Requirements
Must have a minimum Master's degree in Social Work (MSW) from a Social Work accredited school.
Required Certificates and/or Licenses
Must have appropriate licensure from the governing body for practice for the District of Columbia, State of Maryland, and/or the Commonwealth of Virginia, if applicable.
