MDS Consultant, N CA (11 facilities)

$140,000 $170,000 DOE

Road Warrier Position

Job Summary:

Under minimal supervision and with a high degree of independence and critical thinking while being 
hands-on in the RAI  process, the MDS Consultant oversees and directs the Minimum Data Set 
(MDS), Care Area Assessment (CAA) process and the RAI Utilization Guidelines - Patient-Driven 
Payment Model (PDPM) processes within specifically-assigned Generations Healthcare facilities.  In 
close collaboration with facility and clinical leadership partners, relentlessly drives timely and 
accurate resident assessment through working side by side with facility MDS Coordinators and the 
Inter-Disciplinary Team (IDT) to ensure full compliance with governing industry, federal and state 
regulations and integration of/alignment with Generations Healthcare standards, policies, and 
processes.  


Essential Functions 
ï‚· Leads, directs, completes, improves, standardizes, trains, and conducts audits to ensure the 
consistency in the delivery and application of standards in resident assessment across facility 
regions and areas. 
ï‚· Acts as tireless clinical partner by assisting in the completion of the RAI process. Including 
but not limited to: Conducting resident assessment, observation and interview; Care Area 
Assessment (CAA) decision making process; Care plan development, Care plan 
implementation and Evaluation, these are all involved in hands-on approach   This hands-on 
work is in the course of regular operations, in the absence of staff or in the needed skill set 
of existing staff, during times of census swings or as needed with  certain resident(s) 
diagnos(es), and as a teaching and training method with staff.   
ï‚· Ensures strict consistency and compliance with CMS regulations and alignment with the 
organization’s overall objectives and needs. Clarifies any deviation from regulations and 
drives corrective action as needed. 
ï‚· Regularly and systematically visits assigned facilities. Ensures organization and reporting of 
results of facility visits; follows up with written reports and ensures facility adherence and 
prompt follow up to any corrective action.  
ï‚· Regularly establishes and supports development of partnership with facility DONs and 
Nurse Consultants to create a more effective and efficient clinical resource; works to 
understand and actively create systems for skill-sharing,  cross training, and other efficiencies 
that build a robustly competent clinical presence. 
ï‚· Working with individual needs and in collaboration with Clinical Services, local DON, and 
facility leadership, assesses MDS Consultants to ensure development of skill set, improve 
quality of work, and support employee productivity and retention. Works with Director of 
MDS Training and Advancement and local Staff Developer to facilitate training to enhance 
MDS Coordinators’ skills set and knowledge base. 
ï‚· Ensures the local and regional maintenance of appropriate privacy and confidentiality 
protocols that pertain to resident assessment, collaborating on any real or potential breaches 
as appropriate. 
ï‚· Acts as subject matter expert and partner to assist facilities with preparation for facility 
surveys through ongoing audits and implementation of recommendations derived from such 
audits, compliance assessments, mock survey, and follow-up. 
ï‚· Works in close partnership with local MDS Coordinators to gather and present survey exit 
summary information with DON, and Administrator, if available, to advise on findings and 
urgent issues.   
ï‚· Implements MDS and PDPM programs, systems and practices for new GHC facilities; 
works in tandem with Operations as needed during due diligence and ensures hands on 
involvement during eventual acquisition/start up to ensure MDS services are implemented 
audited, and improvements made as needed within the initial days and weeks of operation. 


Knowledge, Skills, and Abilities: 
ï‚· Extensive knowledge of the Minimum Data Set (MDS) and Patient-Driven Payment Model 
(PDPM) as utilized in a Skilled Nursing Facility setting 
ï‚· Extensive knowledge of MDS and PDPM in relationship to Quality Measures and the CMS 
Five-Star Quality Rating System 
ï‚· Knowledge of essential computer programs including, but not limited to, Microsoft Office, 
Adobe, Zoom (or other webinar platform), etc. 
ï‚· Ability to execute on multiple assignments and priorities with extensive detail coordination 
and program management 
ï‚· Strong leadership and influence skills 
ï‚· Demonstrated ability to manage change 
ï‚· Exemplary communication, influencing, consulting, and stakeholder management skills 
ï‚· Ability to interact effectively with a wide variety of personality types

Education and Experience 
ï‚· Degree in Nursing with current and clear RN licensure within the state(s) of practice 
ï‚· 7-10 years’ experience in skilled nursing with an emphasis on patient assessment and MDS 
management 
ï‚· Experience in creating strong relationships that foster the spirit of innovation, improvement, 
and collaborative solutions and developments 
ï‚· Experience in group facilitation required
