Job description

Seeking an RN care coordinator for primary care clinic in Bush Alaska. This position will coordinate care for patients in several remote locations for chronic care appointments, follow up for recent hospitalization, and an advocate to patient needs. Must be culturally sensitive, respectful and courteous. Strong organizational skills, communication skills to care providers and patients, proficient in computer, telephone communication a must. Experience in Care Coordination/Case Manager required. Housing will be provided probable with roommates, so cannot have pets, or S/O. Current COVID and flu vaccine required. Alaska is not a compact state, applicant must have a current and unencumbered license in home state with an Alaska license application.
