In this role, you will work closely with our residents and their families, providing emotional support, assisting with care planning, and helping navigate psychosocial needs in a warm, welcoming environment. It’s a fantastic opportunity to gain hands-on, clinical experience in long-term care.

What You’ll Do:

  • Conduct psychosocial assessments and assist with resident admissions/discharges.
  • Provide emotional support and advocacy for residents and their families.
  • Collaborate with an interdisciplinary team (nursing, therapy, and activities) to enhance resident quality of life.
  • Assist with documentation and community resource coordination.
