Social Worker (BSW) – Kansas Overview

Our client is seeking a dedicated Social Worker to support patients and families across Kansas. In this role, you will coordinate patient care plans, assess healthcare needs, and ensure timely, high‑quality service delivery. The ideal candidate demonstrates compassion, professionalism, and strong communication skills while maintaining dignity, respect, and fairness in all patient interactions.

Key Responsibilities
  • Coordinate individualized plans of care with a focus on quality, timely service delivery, and medical necessity
  • Assess, plan, implement, monitor, and evaluate care options to meet patient healthcare needs and family support systems
  • Ensure coordinated, efficient, and safe care that promotes positive health outcomes
  • Maintain an open, approachable manner while treating patients and families with fairness, respect, and dignity
  • Communicate effectively with interdisciplinary teams to support seamless care transitions
Required Qualifications
  • Bachelor's degree or higher
  • 1+ years of experience in Healthcare
  • 1+ years of experience in ANY of the following:
    • Case Management
    • EHR Coordination
    • Care Management
  • Valid BSW license in the state of Kansas (KS)
