Position Type: Full-time/Part-time

Shifts Available: Days M-F

Job Summary:
Arch Recovery Center is seeking a motivated and compassionate Case Manager to join our team at our Level 3.5 ASAM (American Society of Addiction Medicine) Substance Use Disorder (SUD) residential treatment facility. The Case Manager will play a vital role in supporting clients by providing guidance, connecting them to community resources, and assisting with aftercare planning. This position is integral to the multidisciplinary team, helping to ensure the best possible outcomes for clients throughout their recovery journey.

Key Responsibilities:

  • Meet regularly with clients to assess their needs and connect them to appropriate community resources, including housing, employment, education, and healthcare services.

  • Assist clients in developing individualized aftercare plans to support their long-term recovery goals.

  • Collaborate with clinicians and other members of the multidisciplinary treatment team to ensure a coordinated approach to client care.

  • Participate in client treatment team meetings, providing insights and recommendations to support the client’s progress and recovery journey.

  • Maintain detailed and accurate notes on client progress, appointments, barriers to success, and strengths, ensuring compliance with documentation standards and regulatory guidelines.

  • Monitor client participation in the program, identify potential barriers to success, and work with the team to overcome challenges.

  • Provide advocacy for clients as needed to help them navigate community systems and resources.

  • Assist with crisis intervention when necessary, providing immediate support and referrals.

  • Engage in continuous professional development and training to stay informed about best practices in case management and recovery support services.

  • Develop treatment plans

Qualifications:

  • Bachelor’s degree in a human services field, including but not limited to:

    • Social Work

    • Psychology

    • Sociology

    • Counseling

    • Criminal Justice

    • Human Services

    • Public Health

    • Rehabilitation Counseling

  • Previous experience in case management, addiction services, or working with vulnerable populations is preferred.

  • Strong organizational skills with the ability to manage multiple tasks and priorities.

  • Excellent communication and interpersonal skills, with the ability to build rapport with clients and collaborate with a multidisciplinary team.

  • Empathy, patience, and a genuine commitment to helping individuals achieve their recovery goals.

  • Basic computer skills for documentation and communication purposes.

Physical Requirements:

  • Ability to sit, stand, and walk for extended periods.

  • Ability to lift and carry up to 25 pounds.

  • Ability to perform physical activities such as bending, reaching, and assisting clients with mobility if necessary.
