Compensation: $19-$21/hr

Status: Full-Time, W2 Employee

Schedule: Monday - Friday, 8 AM - 5 PM

About the Role

We are seeking a compassionate and dynamic Bridge Care Coordinator to join our Bridge Program Team. This position is a critical part of enhancing the care experience by bridging the gap between home health and hospice services. You'll help provide peace of mind to families and create meaningful connections with the seniors you serve.

This unique program ensures a smoother transition for our patients by providing consistent, compassionate support, our Bridge Program redefines post-acute care with an added layer of comfort and continuity.

Your key responsibilities will include:

  • Conducting scheduled in-home non-clinical visits with Bridge Patients.
  • Educating families about Aegis Healthcare's continuum of services.
  • Connecting patients to valuable external resources.
  • Bridging the gap between home health and hospice by offering guidance and support to patients and families.
  • Offering companionship that fosters trust and well-being for seniors.
  • Foster and maintain strong, collaborative relationships with facility staff.

Qualifications

To thrive in the role of Bridge Patient Care Coordinator, you must:

  • Be at least 18 years old.
  • At least 1 year in healthcare marketing (preferred).
  • Possess a valid driver's license, clean driving record, and reliable automobile.
  • Provide proof of current automobile insurance (MVR required).
  • Have a clear negative TB test.
