Your Role as a Bridge Care Coordinator
As a Bridge Care Coordinator, you will be an essential part of our team, providing support and companionship to aging patients through our Bridge Program. Your responsibilities will include:
Visiting patients in their homes and providing peace of mind.
Educating families about available healthcare services.
Connecting patients with community resources.
Helping transition patients between care services, including hospice.
What We're Looking For
18+ years old
Healthcare experience required
Strong organizational & computer skills
Ability to work independently
Valid driver's license & clean driving record
Reliable transportation & current auto insurance (MVR check required)
Negative TB test
