Berks Community Health Center is seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join our dynamic team, focusing on hospice care. This full-time position allows for remote work, providing flexibility while supporting individuals in a crucial stage of their lives. This full-time LCSW position is primarily remote, giving you the flexibility to work from your home office while maintaining close communication and collaboration with your team and patients. You will be supported by advanced telehealth technologies, enabling effective patient interaction and documentation. • Establish rapport quickly while effectively communicating with patients, families, Berks Community Health Center staff, and interdisciplinary team members using active listening and clear, jargon-free language.
• Conduct thorough and developmentally appropriate assessments of patients referred to our hospice center, including screenings for mental health issues such as depression, anxiety, and cognitive impairment.
• Utilize electronic medical records efficiently, ensuring detailed documentation and care plan implementation occurs within 48 hours of patient encounters.
• Create and manage a caseload of hospice patients, providing follow-up interventions and referrals as necessary to support their needs.
• Collaborate closely with primary care providers and other team members to develop integrated care plans that address the holistic needs of patients.
• Maintain a high standard of confidentiality and privacy in all patient interactions and documentation.
• Engage actively in supervision, case reviews, and quality improvement processes to enhance care delivery standards.
• Refer patients to appropriate psychiatry consultations for medication management as prescribed by BCHC medical providers.
• Facilitate connections to community resources that align with patient goals, including educational, vocational, and self-help services.
• Fulfill additional responsibilities as needed to enrich patient care and support organizational objectives.

This role focuses on patients in hospice care, where sensitivity and empathy are paramount. The ideal candidate will demonstrate cultural competence and understanding of the dynamics involved in end-of-life care, specifically supporting underserved populations and homeless individuals. Your role is crucial in ensuring that patients feel valued and understood during one of the most vulnerable periods of their lives.

• A Master’s Degree in a related Behavioral Health field.
• Current and valid LCSW licensure.
• Certification in CPR or Basic Life Support (BLS).
• A minimum of three years’ experience working in the behavioral health field, particularly with hospice patients.
• Proficient in trauma-informed care and a strong commitment to social justice principles.
• Excellent verbal and written communication skills and strong problem-solving abilities.
• Ability to navigate and utilize electronic health records effectively for patient documentation and care coordination.

• Experience working with diverse and underserved populations, particularly within hospice settings.
• Bilingual proficiency in English and Spanish is preferred to effectively communicate with a broader range of patients.
• Familiarity with population-based care principles for both preventive and chronic health conditions.

We offer a competitive salary range of USD 90,000 to 100,000 per year, commensurate with experience and qualifications.
