Job description

SUMMARY. The Client Case (CC) Manager has oversight of the youth client's safety, care, social & service support and development. The principal role of the CC Manager is for social & service support. The CC Manager is the client's primary advocate and liaison. The CC Manager is the contact person for the child placement agency's case managers, therapists, schools, employees, other outside agencies or family members. The CC Manager assists with entry of the client to the facility, initial assessment, regular reporting, all health-related appointments and a monthly program plan and any other client related social and service support.
ESSENTIAL FUNCTIONS:
  • Supervised by the Director
  • The Client Case Manager is a role model in behavior and work ethic for the staff and clients
  • Adheres to Avery's Village, LLC. Employee Policies and Procedures manual
  • Completes all required training
  • Manages and responsible for a case load for up to 16 youth clients
  • Liaison with the Child Placement Agencies (KVC, St. Frances, TFI and Cornerstones of Care) for placing clients at Avery's Village and all other client needs
  • Ensures all documentation for clients are received for placement
  • Coordinates the admission of all new clients
  • Conducts an initial assessment of new clients
  • Creates and maintains the case books for clients
  • Requests the 30-day Qualified Residential Treatment Program
  • Ensures clients have completed eye, dental and physical health examinations
  • Sets up medication and/or therapy appointments
  • Writes weekly and monthly progress reports on the clients
  • Creates, monitors and updates a program treatment plan for each client
  • Creates and implements a transition plan and discharge documentation for all clients
  • Provide crisis intervention for clients
  • Make and assists with transportation of clients to appointments
  • Assist clients with school and employment
  • Liaison for communication, facilitation or meetings with CPA's case managers, therapists, schools, employees, other outside agencies, foster families or family member
  • Assist Client Care Tech on the floor with clients if needed
  • Performs other duties assigned by the Director

PHYSICAL DEMANDS:
  1. Frequently move/lift between 25-50 pounds, depending on assignment requirements
  2. Perform all duties that may require sitting, kneeling, squatting and walking
  3. Manual dexterity and fine motor coordination

REQUIRED EDUCATION AND EXPERIENCE:
  1. Candidates must be at least 21 years of age or older
  2. Bachelor's degree from an accredited college in one of the Human Services related fields
  3. 2-5 years previous experience as a case manager
  4. Familiarity with mental health theory and practice
  5. Proficient technology skills utilizing Windows platform, including the Microsoft Office suite (Word, Excel, Outlook, PowerPoint) required

DESIRABLE KNOWLEDGE, SKILLS, and ABILITIES:
  • Current CPR/First Aid Certification
  • Knowledgeable and sensitive in working with culturally diverse staff and clients
  • Solid knowledge in social helping and a positive approach to complex mental health issues
  • Excellent interpersonal and communication skills, including both oral and written.
  • Must have strong skills in organization and planning, demonstrated ability to work independently and problem solve
  • Demonstrated ability to manage multiple people and projects
  • Ability to prioritize tasks, exercise sound judgment and confidentiality with sensitive information
  • Ability to establish effective working relationships with staff, community agencies, and consumers

OTHER POSITION ELIGIBILITY REQUIREMENTS:
  1. If required to operate a company vehicle during the course of employment, must possess a current state driver's license and have an acceptable driving record for the past three (3) years
  2. Ability to pass a criminal background clearance check and a physical exam and TB test.
